Offered for the first time in Canada, this workshop combines principles of population health management with the IHI Triple Aim framework.
In this one-and-a-half day workshop, IHI’s core faculty will introduce you to Triple Aim’s concept design and approach to provide you with practical guidance on setting up the infrastructure to facilitate effective population management. The workshop will also feature examples of the Triple Aim’s application in Canada and the U.S.
What is the Triple Aim?
The Triple Aim framework, developed by IHI, refers to the simultaneous pursuit of better care for individuals, better health for populations, and lower per capita costs and has been piloted with over 100 organizations around the world. Population health management strategies will help you keep your patient population as healthy as possible, minimizing the need for expensive emergency room visits, hospitalizations, and other health services.
